Consumer-Directed Services

Advanced Caregivers In Home Health Care Services, LLC offers Consumer Directed Services that give the client or the family better control over the level of care they get at home (or in their preferred living environment). The family or the client can choose who to hire as their caregiver under the close guidance of our home health agency in the process of selecting their caregiver.

Our Home and Community Based Services (HCBS) program is intended to provide necessary assistance in meeting the unmet needs of seniors and adults with disabilities. More importantly, this program will enable the individuals to remain in their home thus reducing cases of institutional care or long-term admission in a care home.

Consumer Directed Services may be arranged and can include services such as:

• ADL Assistance: Bathing, Hair Washing and Shaving Assistance
• Companionship at Home
• Assist with Home Exercise Program
• Light Housekeeping Tasks
• Keep Track of Appointments / Schedule of Medical Checks
• Ambulation Assistance / Mobility
